Complete and record this form, along with an official copy of the deceased joint tenant's death certificate, to formalize the transfer of property rights to the surviving co-owner.

To remove the decedent's name altogether, execute a new deed showing both joint tenants as grantors and only the survivor as a grantee. For example: "John Doe (deceased) and Richard Roe, as joint tenants with right of survivorship, convey to Richard Roe as sole owner." Present the new deed along with the affidavit of surviving joint tenant or submit it for recording on its own at a later date.

Process is easy but system would not accept 3 different credit cards on first day. No phone number to call. Sent message and response was all 3 cards must have been declined. However, next day one of those cards went through with no problem.

The site is fine with one exception. About half the pdf files I downloaded were corrupted. I could not open them or view their contents. Fortunately, the link continued to work, so after I discovered this, I downloaded the corrupted files again, and they now seem fine. I do not know if my computer or the website caused this odd problem.

We had hoped, as this was direct through our State recorder's office, State-specific data would be pre-filled in. Also there is no help when transferring the home title from a Revocable Trust to the living Trustee and new spouse (no example given, no help for which code to use). And the example doesn't match the prior deed revision format submitted by our attorney. So, not the best experience. We may have to get an attorney involved...what we were hoping to avoid
