In Maine a Notice of Death Affidavit is appropriate for use by a surviving beneficiary identified in a recorded Transfer on Death Deed. The beneficiaries record the Notice of Death Affidavit (in the registry of deeds in the county where the real property is located) (to confirm title following the death of the transferor.)
The Notice of Death Affidavit must contain:
1. (The name and address, if known, of each beneficiary taking under the transfer on death deed)
2. (The street address of the property)
3. (The date of the transfer on death deed)
4. (The book and page number at which the transfer on death deed was recorded prior to the transferor's death)
5. (The name of the deceased transferor)
6. (The date and place of death)
7. (The name and address to which all future tax bills should be mailed)
8. (The affidavit must be notarized.) (ME revised statute 6-414)

It was very easy to order,download, and print. The only issue I have is that the guide that came with my form really did not help me filling it out. I feel the explanations could have been better and suited more for the standard person. I was still confused when filling it out and will probably have to get a lawyer to make sure it's filled out correctly

I had overwhelming emotions taking my deceased wife's name off my condo, so it took me a year to steel myself to submit the form. Although I am sure that the Quit Claim form was acceptable for my county, I felt that it was not formatted in the manor that I have seen while viewing the other deeds recorded. So that forms that I received were not useful to me.
