In Maine a Notice of Death Affidavit is appropriate for use by a surviving beneficiary identified in a recorded Transfer on Death Deed. The beneficiaries record the Notice of Death Affidavit (in the registry of deeds in the county where the real property is located) (to confirm title following the death of the transferor.)
The Notice of Death Affidavit must contain:
1. (The name and address, if known, of each beneficiary taking under the transfer on death deed)
2. (The street address of the property)
3. (The date of the transfer on death deed)
4. (The book and page number at which the transfer on death deed was recorded prior to the transferor's death)
5. (The name of the deceased transferor)
6. (The date and place of death)
7. (The name and address to which all future tax bills should be mailed)

Very good information. Easy access and easy to download. All the forms needed for TOD to be notarized and recorded with the county office. Much better than working with a Trust and the expense of lawyers, especially when several parties are involved and the owner of said property knows exactly to whom the property should go. Having forms and instructions available for the public to have their wishes recorded and confirmed makes handling final planning much easier and prevents family members from having the unnecessary task of going through court to solve property distribution issues. Thank you for this site and the forms you provide. I will recommend Deeds.com to those I know who are making final plans.

Great to have a downloadable form, rather than a cloud solution that gives no guarantee of privacy. Appreciated the sample.......but all of that still left me with open issues about how to tweak the form to serve my particular needs......for example: how to ensure that survivor rights were properly characterized; how far back I should go with the "Source" section + how I should layer my own additions to the chain of ownership, etc. Nonetheless, an overall happy experience. Deeds.com recorded documents for me without any issue. It's a good service and provides a lot of assistance on its web site. However, I asked for a printed receipt (i.e. an pdf copy of one), but after a staff provided me with an obvious answer, they simply ignored my follow up requests. I also asked a simple formatting question that they should have been able to answer; instead, they passed the buck and referred me to the recorder, which currently is a very time-consuming venture. They also have no telephone number for any issue. Generally, the service saves me the time and effort of physically recording a document, but when you think about $19.00 per recording seems like a steep price for the services rendered.
