Use this form to revoke a previously issued Power of Attorney, [General, Special, Durable, Parental, etc] whether recorded or not. If a Power of Attorney was recorded or referenced any (transfer of real estate or any legal or equitable interest therein, excluding leases), then the Revocation Notice must be recorded. (33-41)(01) This Revocation Notice is fully formatted to meet County Recording Requirements. It includes a page that lists where the Revocation Notice was sent for documentation.

Thank you! My husband and I went in the get notary stamps for a Special Warranty Deed and a Post Nuptial Agreement. The representative was very knowledgeable and thorough with the notary process. The Mineral Deed transfer form was pretty good. Could have used more info in the guide about where to find legal property descriptions and source of title. Also more space on the pdf for entering return addresses - there was room for only one; I needed three. I will be sending the form to the County Courthouse soon. I hope it works.
