In Maine a Notice of Death Affidavit is appropriate for use by a surviving beneficiary identified in a recorded Transfer on Death Deed. The beneficiaries record the Notice of Death Affidavit (in the registry of deeds in the county where the real property is located) (to confirm title following the death of the transferor.)
The Notice of Death Affidavit must contain:
1. (The name and address, if known, of each beneficiary taking under the transfer on death deed)
2. (The street address of the property)
3. (The date of the transfer on death deed)
4. (The book and page number at which the transfer on death deed was recorded prior to the transferor's death)
5. (The name of the deceased transferor)
6. (The date and place of death)
7. (The name and address to which all future tax bills should be mailed)
8. (The affidavit must be notarized.) (ME revised statute 6-414)

Very satisfied. Download was easy, completing the form was easy, got our signatures notarized and submitted it to the register of deeds. The only item was that the register of deeds did not immediately recognize the TOD deed form as the usual form they receive. After carefully reviewing all the information and wording on the deed she accepted it.

Deed.com is AMAZING! I only had about 2 weeks to get my quit claim deed recorded by my county office before my refinace due date approached. When I uploaded my quit claim to Deed.com I got it electronically recored by county register's office in "24 hours"!!! I was in despair as I needed to file two (2) very important documents with the County. Due to Covid the office was closed and my only recourse was to E-Fie with a service provider. I was fortunate enough to hear about Deeds.com. They were specific, courteous, patient and most of all productive. My documents will take awhile for the final filing but that is because the County happens to have a slow turn around time. Otherwise, I am now relieved that this part is over. Thank you Deeds.com. Contents were well done. Could not remove and replace the "Deeds/" footer, rendering the form unusable for filing with a court and county deed records. This should be corrected.

Quite simple and easy. Only one critique: It would be easier if the names of the PDF would reflect the name of the deed/form such as 'Controlling tax return' rather than '1579101185SF56863.pdf'. However I love downloading forms rather than mail.
